Disability Services
The Office of Disability Services at SoLAcc serves as an advocate for students with documented disabilities to help them make a smooth transition to college and ensure equal access to the programs, services, activities, and facilities offered by the college.
Various accommodations and support services are in place to assist students according to their documented needs. Students with disabilities are encouraged to apply for Disability Services and request accommodations prior to the beginning of the semester.
- Must have a documented disability or be regarded as having a disability.
- Must self-identify to Disability Services.
- Must submit appropriate documentation prior to receiving services.
To Apply for Disability Services and Request Academic Accommodations:
- Complete and submit an and upload official disability documentation to the form. Refer to our for specific information and criteria required for the documentation.
- Once the required paperwork (Accommodations Request Form, Disability Documentation, and Initial Application) have been received and reviewed, the Disability Services Office will contact the student to schedule a required in-person appointment to finalize the student’s application and accommodations.

Documentation Guidelines and Criteria
Students must submit official documentation of their disability from a licensed professional to apply for accommodations at SoLAcc. Documentation MUST meet the specific guidelines listed below to approve a student for college accommodations.
- The student is responsible for obtaining documentation from a licensed health care/mental health profession who is qualified to diagnose and treat the student’s primary disability.
- Documentation must be submitted BY THE STUDENT with the APPLICATION PACKET. It should not be submitted to Disability Services separately by a provider, clinic, or school for the student.
- The provider should type or print the documentation on official office/clinic letterhead. We do not accept documentation written on prescription pads, patient visit summaries, prescription lists, or printed medical records/histories. We do not provide templates or forms for practitioners to fill out as documentation.
- Documentation MUST be current (within the last 3 years).
- Documentation MUST list the student’s primary diagnosis, as well as, any secondary/other diagnoses that rise to the level of a disability.
- Documentation must clearly describe how the diagnosis is impacting the student’s education and creating a significant barrier for the student to full educational access in his/her academic courses. (The expected impact should go well beyond the mild “test anxiety” experienced by most people in a typical test-taking situation)
- Documentation should include any suggestions for academic accommodations that the provider feels are appropriate for this diagnosis.
- IEP’s and 504 Plans are for K-12 only and do not transfer or apply to college courses. IEP’s and 504 Plans can be submitted for supplemental information (ALL pages of the plan are required); however, they will NOT be determining factors regarding final decisions for accommodations at SoLAcc. Additional documentation and information is generally required for final approval.
- Documentation should be included and submitted with the Application Packet when the student applies for accommodations. We do not accept documentation that is emailed or brought in separately (without a completed Application Packet).
