Disability Services
The Office of Disability Services at SoLAcc serves as an advocate for students with documented disabilities to help them make a smooth transition to college and ensure equal access to the programs, services, activities, and facilities offered by the college.

Documentation Guidelines and Criteria
Students who identify as having a disability for which accommodations are needed must submit official documentation from a licensed professional who is qualified to diagnose and treat the disability.
PLEASE NOTE:
- Documentation must be typed/printed by the provider on official letterhead of the practitioner, clinic, or agency. SoLAcc does not provide templates or forms for practitioners to fill out or complete as documentation.
- Handwritten notes on prescription pads, visit summaries, patient histories, or progress reports are not accepted as documentation.
- Documentation MUST be current (within the last 3 years).
- IEPs and 504 Plans are K-12 plans ONLY. They do not transfer to college and are not used in higher education. While they can be submitted and reviewed for supplemental information, they will NOT be determining factors regarding eligibility and approval for college accommodations.
DOCUMENTATION MUST INCLUDE THE FOLLOWING INFORMATION TO APPLY FOR SERVICES AND ACCOMMODATIONS:
- The student’s primary/secondary medical or mental health diagnoses that rise to the level of disability* and are directly impacting the student’s education.
- Date of diagnosis and the tests, evaluations, assessments, or criteria used to make the diagnosis.
- A clear description of how the student’s disability is creating a significant barrier to full access in their academic courses when compared to their peers. Functional limitations, level/severity of impairment, and expected impact on academic performance must be included.

- Extended Test Time
